AICTS Pune Doctors Perform Rare Dual-Valve Heart Procedure Without Open-Heart Surgery

Specialists at the Army Institute of Cardio-Thoracic Sciences (AICTS) in Pune have achieved a groundbreaking milestone in cardiac care by successfully performing a highly complex and rare minimally invasive heart procedure. This intervention represents a significant advancement within the Armed Forces Medical Services (AFMS).

The procedure, known as bicaval transcatheter valve implantation (CAVI), was conducted on a patient with a previous history of heart valve replacement surgery who subsequently developed severe leakage of the tricuspid valve on the right side of the heart. CAVI involves the simultaneous placement of two prosthetic valves into the superior vena cava and inferior vena cava, the primary veins that return blood to the right side of the heart. By utilizing this innovative catheter-based technique, the medical team effectively treated the patient’s condition without resorting to a repeat open-heart surgery, thereby significantly reducing surgical risks and recovery time.

This achievement highlights the increasing capabilities of military medical institutions in India to perform state-of-the-art structural heart interventions typically available only at a select few specialized cardiac centers around the world. Prior to the intervention, the patient suffered from tricuspid regurgitation, a serious condition that arises when the tricuspid valve fails to close properly, causing blood to flow backward into the body’s venous circulation instead of being pumped efficiently through the heart. This condition often leads to severe symptoms such as leg and abdomen swelling, liver congestion, fatigue, and breathlessness.

Traditional surgical options are considerably more complex and carry greater risks for patients who have undergone previous heart surgeries, primarily due to scar tissue and other complications. Acknowledging these challenges, the multidisciplinary team at AICTS chose a minimally invasive catheter-based approach, successfully implanting two prosthetic valves in the veins leading to the right atrium. These newly placed valves prevent backward blood flow, restoring circulation without subjecting the patient to another major surgical procedure.

Post-surgery, the patient had a successful recovery, underscoring the efficacy of this advanced treatment option for carefully selected high-risk individuals. Medical experts characterize bicaval transcatheter valve implantation as one of the most technically challenging procedures in structural heart disease. While Transcatheter Aortic Valve Replacement (TAVR) has gained widespread acceptance for aortic valve disease globally, CAVI remains an emerging therapy with limited application worldwide.

This procedure is primarily recommended for patients suffering from severe tricuspid regurgitation who are not ideal candidates for traditional surgery due to age, prior cardiac interventions, or multiple health issues. As the international experience with this technique expands, it is increasingly viewed as a viable alternative for managing intricate right-sided heart valve disease.

The successful completion of this intricate procedure at AICTS reflects the rapid advancements of minimally invasive cardiac care within the AFMS. It also underscores the growing proficiency of Indian military cardiologists in adopting advanced catheter-based therapies, providing safer and more effective treatment options for patients previously facing limited alternatives.
